A plain amber bottle, a few capsules and a broccoli floret beside a blank lab sheet.
This article is part of our Male Fertility: The Complete Guide.
Short answer: DIM and TRT can sit in the same routine only if your prescriber knows about it, because the evidence for DIM and TRT together is thin. DIM (diindolylmethane) is a dietary supplement, not an aromatase-inhibitor drug. The human trials we opened tested it in women, and the only data in men are two single-patient case reports. If estradiol runs high on testosterone therapy, the documented route is lab-guided management by your prescriber. Read the checks below, then look at the official DIM 3X page if you still want to try it.

Men on testosterone therapy often ask the same thing in forums: my estrogen is climbing, so can I just add DIM? This guide is part of our male health and fertility guide, and it treats DIM and TRT as a question to answer with labs and a prescriber, not with a bottle alone. We opened PubMed abstracts, StatPearls, Memorial Sloan Kettering, the FDA and the manufacturer page, and we quote only what those pages say.
DIM and TRT in plain English
TRT means testosterone replacement therapy, a prescription treatment for men diagnosed with low testosterone. The Endocrine Society guideline recommends diagnosing hypogonadism only in men who have symptoms and signs of testosterone deficiency plus unequivocally and consistently low morning testosterone on repeat testing (PubMed 29562364). That is a medical diagnosis, not a supplement-aisle decision.
DIM is short for diindolylmethane, a compound formed when the body breaks down indole-3-carbinol from cruciferous vegetables such as broccoli. Our explainer what is DIM covers the basics, and DIM supplement for men covers the product category. Here the narrow question is how DIM and TRT interact in real life, and the short version is that science has not tested it.
Check 1: what the research on DIM and estrogen actually shows
Most of the human data on DIM come from women, because the research interest was breast health. Three papers show how far that evidence is from a man on TRT:
- In a randomized, placebo-controlled trial, 130 women prescribed tamoxifen took 150 mg of a DIM product twice daily or placebo for 12 months. The urinary 2/16α-hydroxyestrone ratio rose with DIM, and SHBG rose by about 25 nmol/L versus about 1 nmol/L with placebo. Tamoxifen metabolites, including endoxifen, were lower with DIM, and the authors called for more research on whether that blunts tamoxifen (PMID 28560655).
- In a 30-day randomized trial of 60 premenopausal women, 75 mg of DIM a day did not raise the estrogen metabolite ratio at day 30 (p=0.05), although body fat percentage fell more in the DIM group (p=0.04). The authors concluded the dose was ineffective for the ratio and that more work is needed (PMID 36111381).
- The National Cancer Institute fact sheet on cruciferous vegetables, reviewed in 2012, says human studies of the related compound indole-3-carbinol show mixed results, and it does not discuss DIM supplements (NCI).
None of that tells a 45-year-old man on weekly injections what DIM will do to his estradiol. We searched Europe PMC on November 8, 2026 and did not find a randomized trial of DIM in men on TRT. If you have seen a claim that the evidence is settled, ask where the men were. Until that trial exists, any claim about DIM and TRT together is an extrapolation.
Check 2: why estradiol can rise on TRT, and who should manage it
Testosterone is converted to estrogen by the aromatase enzyme. StatPearls explains that this aromatization can cause hyperestrogenism in men on therapy, advises periodic estradiol checks, and says men with breast symptoms such as tenderness, enlargement, pain or nipple sensitivity should have estradiol measured (StatPearls, last updated Feb 25, 2024). It also says aromatase inhibitors may be needed in some cases, which is a prescriber call.
A 2021 review from a high-volume sexual medicine practice shows how rare and how clinical this is. Of 1,708 men placed on testosterone therapy, 44 (2.6%) had elevated estradiol that was treated with anastrozole 0.5 mg three times a week. Median estradiol fell from 65 to 22 pg/mL while total testosterone stayed similar, 616 versus 596 ng/dL (PMID 34090245). The thresholds in that practice were 60 pg/mL or higher, or 40 to 60 with symptoms. Those are one clinic’s rules, not a self-treatment guide.
The takeaway for DIM and TRT: if your estradiol is truly high, a prescriber has tools with published numbers behind them. A supplement with limited data in men is not a like-for-like swap, and starting one before a lab result can hide the picture your doctor needs.
Check 3: the labs to request before you add DIM
Before combining DIM and TRT, ask what your clinic already tracks, then add a baseline before any supplement so a change can be traced. The sources we read point to the following list.
| Test | Why it matters on TRT | Where we read it |
|---|---|---|
| Total testosterone | Confirms the dose is doing what the prescriber intends; usually drawn in the morning | MedlinePlus, updated June 5, 2025 |
| Estradiol | Checked in men with breast symptoms and periodically, because aromatization can raise it | StatPearls |
| Hematocrit and hemoglobin | Testosterone can raise red-cell counts; above the upper normal limit therapy is not recommended or the dose is reduced or stopped | StatPearls |
| PSA | Drawn before starting in men over 40 and followed afterwards; a PSA above 4 ng/mL is listed as a contraindication | StatPearls |
MedlinePlus also reminds readers that an abnormal result alone does not diagnose a condition. Good labs are a conversation starter with your prescriber, nothing more. If fertility is on your mind, read how to track a male fertility supplement with semen analysis first, because the Endocrine Society recommends against starting testosterone therapy in men planning fertility in the near term.
Check 4: DIM is not an aromatase-inhibitor drug
The FDA states that it does not approve dietary supplements for safety or effectiveness before they are sold, and that a product intended to treat, prevent, cure or alleviate a disease is a drug even if the label says supplement (FDA 101: Dietary Supplements). That legal line explains why the DIM 3X page carries the standard statement that it is not intended to diagnose, treat, cure or prevent any disease.
So when you compare DIM and TRT support options, you are comparing a prescription drug class that went through approval with a supplement that did not. The same FDA page warns that supplements can interact with medicines and can interfere with lab tests, which matters for anyone doing DIM and TRT together, because your treatment is judged by bloodwork.

Check 5: medicine interactions and the tamoxifen signal
Memorial Sloan Kettering, in a page last updated December 15, 2023, says laboratory studies suggest anticancer effects but human data are limited, and that DIM may affect hormone levels. It adds that DIM may make medicines handled by CYP450 enzymes, or moved by MDR1, less effective, and it recalls the study where endoxifen fell (MSK: Diindolylmethane).
Most men on TRT are not on tamoxifen, but the lesson carries over: a supplement that changes how the liver clears drugs is not neutral. If you take a statin, a blood thinner, a blood-pressure pill or anything else on a schedule, bring the DIM bottle to a pharmacist before the first capsule. That is why DIM and TRT users should ask a pharmacist, not a forum.
Check 6: what the DIM 3X page says versus the research
We opened the manufacturer page on November 8, 2026. Below is what the manufacturer states, set against what the sources above show for DIM and TRT users. We repeat the claims only as claims.
| The manufacturer states | What the sources we opened say |
|---|---|
| It blocks the aromatase enzyme | No trial in men on TRT found. MSK says human data on DIM are limited. |
| It converts “bad” estrogen to “good” estrogen metabolites | A 12-month trial in women on tamoxifen raised the 2/16α ratio; a 30-day, 75 mg trial in premenopausal women did not. |
| No negative side-effects, no doctor visits needed | MSK lists case reports (visual impairment, rash, stroke, pulmonary embolism with DVT) linked to DIM supplements. TRT itself needs lab follow-up. |
| Benefits could start in 2 weeks | Trials ran 30 days and 12 months in women; none measured how men feel. |
| 1 capsule daily with AstraGin, BioPerine and vitamin E; 67-day guarantee, refund minus shipping | A label and policy fact. Read the Supplement Facts panel and the refund terms yourself before ordering. |
Dose matters when you read the studies. The trials above used 75 mg a day and 150 mg twice daily, so compare the DIM per capsule on the Supplement Facts panel with those numbers rather than trusting a headline. Our own DIM3X questions page goes through the label line by line, and DIM vs I3C explains how the two compounds differ.
Check 7: what men on DIM and TRT should watch and who should skip it
Two single-patient case reports are the only male data we opened, and both are weak evidence. A 2026 report followed a 78-year-old man who took 100 mg of DIM daily for three months after stopping other supplements; his PSA fell from 4.6 to 2.4 ng/mL and testosterone rose from 436 to 615 ng/dL, but the authors say a single case needs larger studies (PMID 41056390). A 2025 report of a 47-year-old man found that a calcium D-glucarate and DIM combination normalized his estradiol while pushing two estrogen metabolites above range, and a further supplement was added to fix a ratio (PMID 41103795). Neither involved TRT.
Anyone combining DIM and TRT should check with the prescriber first, and so should anyone who takes any prescription medicine, have a hormone-sensitive condition, or are scheduled for surgery. Memorial Sloan Kettering advises pregnant and nursing women not to take DIM. Stop the supplement and seek care if something feels wrong, and report serious reactions to the FDA, as the FDA page describes. Our CortiSync vs DIM3X comparison and cortisol and testosterone guide show how stress hormones can muddy the same symptoms, and cortisol and testosterone supplements looks at that side.
A short checklist for DIM and TRT at your next prescriber visit
- Bring the exact DIM 3X or other DIM label and say how much you plan to take.
- Ask whether your latest estradiol, hematocrit and PSA are in range for you, and when the next draw is.
- Ask whether DIM could change SHBG or how your testosterone result is read. That question comes from the tamoxifen trial in women, not from a finding in men.
- Agree on one change at a time, with a repeat lab, so a result can be traced to its cause.
- Keep the bottle and receipt for the 67-day window the manufacturer states, and note the date you start.
Verdict: DIM and TRT, keep it prescriber-led
If you decide to try DIM and TRT together, start with a baseline lab, change one thing at a time and write down the dates.
Our verdict on DIM and TRT is cautious. DIM may be worth a conversation, and the manufacturer’s guarantee lowers the money risk, but the safety and effect on men taking testosterone are unstudied. Treat any DIM and TRT plan as a lab-tracked experiment you run with your prescriber, not a fix you run alone. Our 2026 bonus-bottle explainer covers the bundle offer if price is your question.

Frequently asked questions about DIM and TRT
Can you take DIM and TRT together?
There is no rule that forbids it, but there is also no controlled trial of DIM in men on testosterone therapy that we could find. DIM is a dietary supplement and TRT is a prescription treatment that needs lab monitoring, so tell your prescriber before adding DIM, and do not change your testosterone dose on your own.
Does DIM lower estrogen in men, and what does that mean for DIM and TRT?
The honest answer is that nobody has shown it well. The human trials we opened were in women. One 12-month trial in women on tamoxifen shifted urinary estrogen metabolites, and a 30-day trial of 75 mg a day in premenopausal women did not raise the metabolite ratio. The only male data we found are two single-patient case reports.
Is DIM the same as an aromatase inhibitor like anastrozole?
No. Anastrozole is a prescription aromatase inhibitor, and a 2021 clinic review of men on testosterone therapy found it lowered median estradiol from 65 to 22 pg/mL in 44 treated men. DIM is a dietary supplement that FDA does not approve before sale, and Memorial Sloan Kettering says human data on it are limited.
Could DIM interfere with my TRT dose?
No study in men on TRT has measured that, so nobody can say. In women on tamoxifen, DIM raised sex hormone-binding globulin (SHBG). Whether that matters for a man on testosterone is an open question to raise with the prescriber who reads your labs.
How do I know if my estradiol is too high on TRT?
You cannot tell from symptoms alone. StatPearls says estradiol should be checked in men with breast symptoms such as tenderness or enlargement, and periodically during therapy. Ask your prescriber to order the test and interpret it in context. Do this before judging any DIM and TRT experiment.
Which blood tests matter when you take DIM and TRT?
The monitoring list in StatPearls includes testosterone, estradiol, hemoglobin, hematocrit and PSA. MedlinePlus notes that testosterone is usually drawn in the morning. Your prescriber sets the schedule.
Can DIM affect other medicines?
Possibly. Memorial Sloan Kettering says DIM may make drugs handled by CYP450 enzymes less effective, and it describes a tamoxifen study in which endoxifen levels fell. Bring every medicine and supplement you use to a pharmacist or prescriber.
How long does DIM take to work?
The manufacturer page we read on November 8, 2026 says benefits could begin in as little as 2 weeks. The trials we opened ran 30 days and 12 months and measured urine and blood markers in women, not how men feel on TRT.
Is DIM 3X safe to take on TRT?
We cannot confirm that. The manufacturer states there are no negative side-effects, while Memorial Sloan Kettering lists case reports linked to DIM supplements, including a stroke, a pulmonary embolism with DVT and an eye condition. Safety on TRT specifically has not been studied that we could find.
