Taking NMN with metformin is a question to settle with your own clinician.
This article is part of our NMN & NAD+: The Complete Guide.
Answer first: NMN with metformin: no human trial we opened tested the two together, so no one can promise it is safe or unsafe for you. Metformin is a prescription drug, so ask your prescriber or pharmacist first. If you and your clinician choose NMN, GenuinePurity NMN is the option we recommend.

People on metformin often hear that NMN is a longevity supplement and wonder if the two can share a daily routine. The honest answer for NMN with metformin is that the human evidence is thin, and the useful part of this page is knowing what to ask.
We read the abstracts and official pages listed under Sources on 2026-10-21. Where a source did not say something, we say so instead of guessing. For the basics of the molecule, start with our NMN and NAD+ guide and is NMN the same as NAD.

NMN with metformin, fact 1: no human trial we opened tested the pair
We looked for a human study that gave NMN to people taking metformin and measured what happened. We did not find one in the sources we opened. The table lists what we did read and what each study cannot tell you.
| Study (source) | Who and how long | What the abstract reports | What it cannot tell you |
|---|---|---|---|
| 2021 trial in prediabetes | Postmenopausal women with prediabetes and overweight or obesity; 10 weeks; randomized, placebo-controlled, double-blind | Insulin-stimulated glucose disposal and muscle insulin signaling increased after NMN, not after placebo | The abstract does not say if anyone took metformin; women only; short |
| 2023 trial in diabetes | 14 men aged 65 or older with diabetes and reduced grip strength or walking speed; 24 weeks; 250 mg a day | Tolerable, no severe adverse events; no difference in grip strength or walking speed | Tiny sample; abstract does not name diabetes drugs |
| 2022 trial in healthy men | Healthy older men; 6 or 12 weeks; 250 mg a day | Well tolerated; blood NAD levels rose | Healthy people, so no medication question |
| 2025 meta-analysis | 12 randomized trials, 513 participants | NAD levels rose; most clinically relevant outcomes were not significantly different from control | Authors flag risk-of-bias concerns and possible exaggeration of benefits |
| 2022 rat study | Rats with induced cerebral hypoperfusion | Examined metformin, rapamycin and NMN each on cognition | Animal model, not a human interaction test |
Our NMN clinical evidence ranking explains how we weigh trials like these.
NMN with metformin, fact 2: why the question comes up
Both products are discussed in connection with glucose handling. The 2021 trial reported better muscle insulin sensitivity after NMN in women with prediabetes. That is interesting, and it is also why a prescriber may want to watch your numbers.
The meta-analysis reached a cooler conclusion: NAD levels rose, but most clinically relevant outcomes did not differ from control. So NMN with metformin is not a proven upgrade, and it is not a proven problem. It is an open question. See NMN and insulin sensitivity for more.
NMN with metformin, fact 3: animal studies are not a human answer
Some web pages cite rodent work to suggest NMN and metformin are a perfect pair. The rat study we opened examined each compound in a brain-injury model. It does not show what happens when a person takes both capsules and tablets daily.
The same caution applies to our own NR and NMN stacking page: lab mechanisms are a reason to run trials, not a reason to stack. Mouse work on gut conversion (PubMed) is covered in NMN vs niacinamide.
NMN with metformin, fact 4: the metformin risks stay the same
Adding NMN does not remove the usual metformin precautions. The NIH Office of Dietary Supplements says metformin might reduce vitamin B12 absorption and lower blood levels. NMN is not vitamin B12.
MedlinePlus on metformin says to tell your doctor and pharmacist about vitamins and supplements, and warns that metformin may rarely cause lactic acidosis. Risk factors it lists include kidney, heart or liver disease, age over 65 and heavy alcohol use. We found nothing saying NMN changes those risks either way.
NMN with metformin, fact 5: NMN is not high-dose niacin
The NIH niacin fact sheet for professionals says large doses of nicotinic acid, about 1.5 g a day or more, are most likely to raise blood glucose, and people on antidiabetes drugs should have glucose monitored.
That passage names nicotinic acid, a different compound from NMN, so do not read it as a warning or a clearance for NMN. It does show why a glucose-lowering drug plus any vitamin-B3 relative deserves a clinician check. Our NMN vs niacinamide page explains the differences.
NMN with metformin, fact 6: diabetes safety data is small
The best human safety signal for people with diabetes is the 2023 study: 14 older men, 24 weeks, 250 mg a day, tolerable with no severe adverse events. That is reassuring but small, and it is not an interaction study.
For general tolerability, see NMN side effects and why people stop taking NMN. For doses, see how much NMN to take daily and when to take NMN.
NMN with metformin, fact 7: check the label and product page first
If your clinician is open to NMN, compare the product page with the bottle. Here is what the manufacturer states on its official NMN page, read on 2026-10-21, and what to verify yourself.
| Item | The manufacturer states | What to verify |
|---|---|---|
| Strength and count | 250 mg NMN per capsule, 60 capsules per bottle | Matches your bottle; ask if 250 mg suits you |
| Purity claim | 96% pure NMN | Batch certificate; see NMN purity testing |
| Serving | 1 capsule daily, preferably with meals, or as directed by a medical professional | Timing relative to metformin: ask your pharmacist |
| Testing | Third-party tested; certificates of analysis linked on the page | Open the certificate and match the lot |
| Price | $69 for 1 bottle, $65 each for 3, $59 each for 6, $49 per bottle on subscription | Per capsule at one a day: about $1.15, $1.08, $0.98 and $0.82 (our math from stated prices) |
| Guarantee | 97-day money-back guarantee; another passage mentions 90 days | Confirm the current refund window before buying |
| Medication warning | The page we read lists no medication-interaction warning | Ask your prescriber; do not rely on the page |

Who should not self-start NMN with metformin
- Anyone who has not told the prescriber that manages the metformin. Start the conversation first.
- People who also use insulin or other glucose-lowering drugs; any change in your glucose picture should be monitored by your clinician.
- People with kidney, heart or liver disease, or heavy alcohol use, which MedlinePlus lists as lactic acidosis risk factors for metformin.
- Anyone with surgery planned; NCCIH advises telling providers about supplements well ahead of an operation.
- Pregnant or breastfeeding people, and anyone under 18: we found no human safety data in the sources we opened.
Questions to bring to your prescriber
- Is any NMN dose reasonable for me given my kidney, liver and glucose results?
- Should I check blood glucose more often for a few weeks, and how?
- Should my vitamin B12 be tested, since metformin might lower it?
- Should I take the capsule with the same meal as metformin or at a different time?
- Which signs mean I should stop NMN and call you?
NMN with metformin: our verdict
Verdict: Treat NMN with metformin as a decision for you and your prescriber, not a hack. The evidence we opened shows NMN raises NAD levels and looked tolerable in small trials, but it never tested the pair. If your clinician agrees, GenuinePurity NMN matches the 250 mg-a-day trial dose and the manufacturer lists third-party certificates.
NMN with metformin: frequently asked questions
Can you take NMN with metformin?
There is no human trial in the sources we opened that tested NMN with metformin together, so nobody can promise it is safe or unsafe for you. Metformin is a prescription medicine. Tell your prescriber or pharmacist about the NMN product before you start, and let them decide.
Does NMN lower blood sugar?
A 10-week trial in postmenopausal women with prediabetes reported that NMN increased insulin-stimulated glucose disposal in muscle. A meta-analysis of 12 trials found most clinically relevant outcomes were not significantly different from control. That is why glucose is worth monitoring with your clinician.
Does NMN interfere with metformin?
We found no human study that measured an interaction, and we will not invent one. Absence of data is not proof of safety. Because both may influence glucose handling, a prescriber may want to watch your numbers if you add NMN.
Is NMN safe for people with diabetes?
One 24-week placebo-controlled study in 14 older men with diabetes (250 mg a day) reported NMN was tolerable with no severe adverse events. It was tiny and short, and it does not replace advice from the clinician who manages your diabetes.
Should I take NMN and metformin at the same time of day?
No study we opened tested timing. The GenuinePurity page we read says to take one capsule daily, preferably with meals, or as directed by a medical professional. Ask your pharmacist how to schedule both products.
Can NMN replace metformin?
No. MedlinePlus says not to change or stop diabetes medicines without talking to a provider, and no trial we opened tested NMN as a replacement for any diabetes drug.
Does metformin affect vitamin B12, and does NMN fix that?
The NIH Office of Dietary Supplements says metformin might reduce vitamin B12 absorption and lower blood levels. NMN is not vitamin B12, so it is not a fix. Ask your prescriber whether a B12 check makes sense for you.
What NMN dose did the human trials use?
The 2022 trial in healthy older men and the 2023 trial in older men with diabetes both used 250 mg a day. Trial doses are not a recommendation, especially when you also take a prescription drug.
Is NMN with metformin good for anti-aging?
No trial we opened tested that combination for aging outcomes. The 2024 meta-analysis of 12 trials says an exaggeration of NMN benefits may exist in the field, so treat longevity promises with caution.
What should I tell my doctor about NMN?
Bring the bottle: name, strength per capsule, how many you take, and when. Mention every other supplement too. MedlinePlus advises telling your doctor and pharmacist about all vitamins, supplements and herbal products you take.
